As a vendor/contractor/consultant doing business with the State University of New York at Fredonia (Fredonia), proof of insurance, NYS Worker's Compensation and NYS Disability Insurance are required prior to commencing all work. Scroll to the bottom of this page to view the insurance matrix applicable to the type of procurement that requires insurance; these matrix's will assist in determining the amounts and types of coverage that is required. These Matrix's are excerpts from The Council of Contracting Agencies and New York State Procurement Council Guidelines for Insurance Requirements in Contracts, adopted by the Council of Contracting Agencies on 12/14/10, adopted by the NYS Procurement Council 6/15/11, updated October 2015. Both Worker's Compensation and Disability Insurance requirements are administered by the NYS Worker's Compensation Board (WCB). All vendors/contractors must contact the WCB and provide the campus with appropriate documentation to show compliance with or exemption from the requirements. Information specific to Worker's Compensation and Disability Insurance can be found here.
Insurance companies must be authorized to do business in New York State. Perform an Insurance Company Search to verify the carrier's authority and that they are listed for the appropriate lines of coverage that they are providing.
