Please take the time to review plan options available to you. Be sure to compare biweekly premium cost, co-pays, deductibles, and prescription coverage costs. Your current health insurance carrier will notify you directly, of any changes in co-pays or benefits each plan year. For your convenience, Health Insurance information books are also available for pick up in the Human Resources Office.
Eligible employees have the option of choosing either The Empire Plan or a NYSHIP Health Maintenance Organization (HMO) for health insurance coverage. To be eligible for coverage under a NYSHIP HMO, you must live or work in the HMO’s NYSHIP service area. The State University of New York at Fredonia is located in Chautauqua County. Click here for a list of the counties each HMO serves. If you no longer live or work in the NYSHIP service area in which you are currently enrolled, you MUST change to another Plan. Employees commuting from outside our immediate Western New York Region may wish to contact Human Resources for additional HMO options/rates.
